Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility located in Olivia, MN. This full-time role offers a $5,000 sign-on bonus and tuition reimbursement, with additional Part Time or PRN positions also available.

You will plan, organize, and direct OT services, conduct resident screenings, develop treatment plans, and collaborate with an interdisciplinary team to ensure high-quality resident care and proper documentation and billing.

Qualifications

• Must hold an active Occupational Therapy license in the practicing state(es).
• Degree in Occupational Therapy.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine optimal interventions and treatments.
• Evaluate residents to develop and adjust treatment plans and goals.
• Interpret and communicate evaluation findings to residents, families, and team members.
• Establish therapy schedules with input from other team members.
• Deliver OT services per plan and ensure proper documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and reimbursement practices are followed
