Job summary

Evolve Therapy Services is seeking a licensed Occupational Therapist for an in-house Skilled Nursing Facility located in Olivia, MN. This full-time, part-time, or PRN opportunity includes a $5,000 sign-on bonus and tuition reimbursement to support your continued education.

You will plan, organize, and direct occupational therapy services, screen residents, evaluate needs, develop short- and long-term goals, communicate with families and the interdisciplinary team, and oversee COTA to ensure

Qualifications

• Must have an Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy and ongoing continuing education.
• Commitment to high-quality resident care and regulatory compliance.

Responsibilities

• Screen residents to determine best intervention and treatment.
• Develop and implement individualized treatment plans with goals.
• Communicate findings and plans to residents, families, and team.
• Create therapy schedules in collaboration with the care team.
• Deliver therapy per plan and document and bill services.
• Oversee COTA and ensure proper documentation and reimbursement.
