Job summary

Neighborhood Healthcare is seeking an Occupational Therapist to deliver patient-centered OT services for seniors, developing mobility-based care plans and supervising a care team on site in Murrieta, CA. You will assess needs, design goals, and adjust therapy to maximize independence and safety.

Minimum CA OT license and current BLS required; experience with elderly populations preferred. A signing bonus of $5,000 is available, and pay reflects local market rates for hourly OT roles.

Qualifications

• CA OT license in good standing.
• One year experience with frail/elderly population.
• Current BLS certification required on hire; CPR/First Aid per CMS/DHCS rules.

Responsibilities

• Develop and implement OT programs for patients in day services.
• Assess patients to determine mobility and functional abilities.
• Create and monitor individualized plans of care to maintain or regain independence.
• Supervise support staff to ensure plan adherence.
• Educate patients and caregivers on adaptive devices and self-care activities.
• Record treatment plans and quarterly progress notes.
