Job summary

Evolve Therapy Services, LLC seeks an Occupational Therapist for an in-house Skilled Nursing Facility in Olivia, MN. You will plan, organize, and direct OT services to ensure high-quality resident care in a facility setting.

Responsibilities include screening residents, developing treatment plans, communicating findings to residents and families, scheduling therapy, and supervising COTA with proper documentation and billing.

Qualifications

• Must hold an active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.
• Degree in Occupational Therapy required.

Responsibilities

• Screen residents to determine best intervention and treatment.
• Evaluate residents to inform treatment plans and goals.
• Communicate evaluation findings to residents, families, and team.
• Establish individual therapy schedules with team members.
• Document and bill occupational therapy services per plan.
• Supervise COTA; ensure documentation and treatments meet standards.
