Job summary

Occupational Therapists Home First Neighbourhood Team in Oxfordshire County Council is seeking a dedicated OT to support adults to regain independence and live safely at home after hospital discharge. You will perform functional assessments, plan reablement, and work with multidisciplinary teams to promote wellbeing and choice.

The role emphasises person-centred care, risk-balanced independence, and leading a caseload within the Home First and Discharge to Assess pathways.

Qualifications

• Qualified Occupational Therapist with HCPC registration.
• Strong communication and collaborative skills.
• Ability to balance independence with risk management.

Responsibilities

• Complete timely assessments and identify barriers to independence.
• Manage and prioritise your own caseload and conduct community visits.
• Collaborate with social workers, discharge colleagues, families and carers.
• Contribute to Home First and Discharge to Assess pathways.
