Job summary

Civic Recruitment Limited is seeking an Occupational Therapist for the Reablement at Home Service in Hackney, London. You will deliver specialist OT interventions, undertake comprehensive assessments and develop therapy-led reablement plans to maximise independence following hospital discharge.

You will work with service users, carers and multidisciplinary teams to promote strengths-based practice and reduce ongoing social care needs, in a 5-month contract with a local authority.

Qualifications

• Degree in Occupational Therapy and HCPC registration are essential.
• Experience in adult social care, reablement or discharge-to-assess services is required.
• Strong report writing, assessment and analytical skills are essential.

Responsibilities

• Undertake comprehensive OT assessments for adults with complex needs.
• Develop personalised Reablement Therapy Plans with agreed outcomes.
• Support independence in daily living and living skills.
• Collaborate with multidisciplinary teams and reablement providers.
• Monitor progress and adjust plans to reduce long-term care needs.
• Provide guidance to reablement teams and carers.
• Ensure documentation is accurate, timely and compliant.
• Support hospital discharge pathways and safe discharge planning.
