Job summary

Sanctuary Personnel is seeking a Locum Occupational Therapist for the Home First Team in Wareham, Dorset. The role is ongoing and full-time with hybrid working options, focusing on discharge planning, reablement, and community OT across Dorset.

You will deliver assessments, coordinate with multidisciplinary teams, and support patients to regain independence while managing a caseload in community settings.

Qualifications

• Degree in Occupational Therapy with HCPC registration.
• Minimum 3 years post-qualified UK OT experience.
• Experience in hospital discharge pathways and reablement services.
• Access to a car and willingness to travel across Dorset.
• Enhanced Adult DBS and Right to Work in the UK.
• Experience within Home First services and community OT.
• Experience working across health and social care systems.

Responsibilities

• Support hospital discharges and discharge planning for seamless transitions.
• Deliver reablement-focused OT interventions to promote recovery and independence.
• Complete comprehensive OT assessments in community settings.
• Collaborate with health and social care partners in multidisciplinary teams.
• Manage a community-based caseload and perform home visits across Dorset.
• Maintain accurate records and case documentation for outcomes.
