DayToDay Health is seeking an Occupational Therapist to provide in-home OT services for home health and hospice patients. You will assess functional status, develop treatment plans, and support patients in maintaining safety, independence, and quality of life in their home environment.

You will educate patients, families, and caregivers, collaborate with an interdisciplinary care team, and document clinical care accurately in an EMR system while traveling to patients' homes.

Competenze

• Licensed OT with state licensure.
• Strong clinical judgment and patient education skills.
• Ability to work independently in home/community settings.
• Experience with EMR documentation.
