Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house skilled nursing facility in Kenosha, WI. This PRN position focuses on planning, organizing, developing, and directing Occupational Therapy Services to maintain high-quality resident care.

Responsibilities include screenings, treatment plan execution, communicating findings, scheduling, documentation, and CMS-compliant charting. Active OT/Licensure and ongoing CE are required.

Qualifications

• Associate's degree in occupational therapy is required.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plans to residents and follow short and long-term goals established by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and families/caregivers and team.
• Establish individual resident therapy schedules in cooperation with team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing.
• Perform all duties to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point of care documentation.
