Evolve Therapy Services in Burlington, WI seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This PRN position focuses on planning, directing therapy services to residents within federal/state guidelines.

Responsibilities include screening, implementing treatment plans, documenting services, and communicating with residents and families. Requires an Associate's degree in Occupational Therapy and active OTA licensure, with ongoing CE to maintain licensure.

Qualifications

• Associate's degree in Occupational Therapy
• Active OTA licensure in state(s) of practice
• Ongoing continuing education to maintain licensure and certification

Responsabilités

• Screen residents to determine best intervention per scope of practice
• Implement treatment plans aligned with short- and long-term goals
• Communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team
• Establish individual resident therapy schedules with other team members
• Document therapy services and ensure billing per plan
• Perform other duties as assigned to effectively treat patients
• Comply with CMS documentation standards of practice daily
