Job summary

Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Beloit, Wisconsin.

Full-time, part-time, or PRN positions available. The role focuses on planning, organizing, developing and directing Occupational Therapy Services to ensure resident care meets CMS standards. Active OTA licensure required and ongoing continuing education.

Qualifications

• Requires an earned OTA degree and active state licensure.
• Demonstrates commitment to ongoing professional development.
• Maintains licensure and certifications as required.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plans and coordinate short and long-term goals.
• Interpret and communicate evaluation findings to residents and team.
• Establish individual resident therapy schedules.
• Deliver therapy per treatment plan and ensure proper documentation and billing.
• Perform duties to effectively treat patients.
• Comply with CMS documentation standards daily.

Education

Associate's degree in occupational therapy

OTA licensure in state(s) of practice

Continuing education to maintain licensure and certification
