Job summary

Evolve Therapy Services in Glendale, WI is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. The role offers Full Time, Part Time, or PRN options and requires an active OTA license and an associate degree.

You will plan, organize, develop, and direct Occupational Therapy Services, screen residents, implement treatment plans, document and bill per CMS standards, and collaborate with an interdisciplinary team.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plans according to established goals and plan of care under supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice

Continuing education to maintain licensure and certification
