Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Glendale, WI. The role supports patient-focused OT services in a facility setting and offers full-time, part-time, or PRN schedules.

Responsibilities include screening, planning, and implementing therapy, coordinating with a multidisciplinary team, and ensuring accurate documentation and billing in line with CMS standards.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment based on scope of practice.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of OT services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.

Education
