Job summary

Evolve Therapy Services seeks a Certified Occupational Therapy Assistant to provide in-house occupational therapy services at our partner skilled nursing facility in Rice Lake, Wisconsin.

The role involves planning and directing OT services, coordinating with the interdisciplinary team, documenting all evaluations and treatments, and ensuring CMS compliance. PRN scheduling allows flexible hours while maintaining high quality resident care.

Qualifications

• Active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy completed.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine the best intervention based on scope of practice.
• Perform treatment plan activities; follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families and team.
• Establish therapy schedules in cooperation with team members.
• Conduct therapy according to plan and document/bill OT services accurately.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.
