Job summary

Evolve Therapy Services in Kenosha, WI, seeks a Certified Occupational Therapy Assistant (COTA) for an in-house Skilled Nursing Facility. This PRN role supports the therapy program, conducts screenings, participates in treatment plans, and ensures documentation and billing compliance.

Qualifications

• Must have OTA licensure in state(s) of practice.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Implement treatment plans under supervision of therapist and follow goals.
• Communicate evaluation findings and recommendations to residents, families, and team.
• Establish resident therapy schedules with the care team.
• Document and bill occupational therapy services per plan and CMS standards.
• Perform all other duties as assigned to support patient care.
