Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. This full-time position includes a $5,000 sign-on bonus, with additional Part Time or PRN options and flexible scheduling.

The successful candidate will plan, organize, develop, and direct OT services per applicable standards, ensuring high-quality resident care and effective collaboration with the interdisciplinary team.

Qualifications

• Must hold an OTA licensure in the state(s) of practice.
• Associate's degree in Occupational Therapy is required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Implement treatment plans and progress toward goals.
• Communicate evaluation findings and plans with residents, families, and the team.
• Schedule resident therapy with other team members.
• Ensure documentation and billing of OT services.
• Perform additional duties as assigned.
• Adhere to CMS documentation standards.
