Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, Nebraska. This Full Time position includes a $5,000 sign-on bonus, with Part Time or PRN options and flexible scheduling.

You will plan, organize, develop, and direct OT services in the facility to meet federal, state, and local standards, and to ensure the highest quality resident care is maintained.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Develop and follow treatment plans with short- and long-term goals as directed by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Deliver therapy according to established treatment plan and ensure proper documentation and billing of OT services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards with daily point-of-care documentation.
