Job summary

Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. Full-time, part-time, or PRN positions with flexible scheduling are available.

Responsibilities include screening residents, implementing treatment plans under supervision, coordinating with a multidisciplinary team, documenting care, and ensuring CMS compliance. Active OTA licensure and an Associate degree are required, with ongoing CE for licensure

Qualifications

• Associate's degree in Occupational Therapy, and active OTA licensure in the state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best interventions.
• Follow treatment plans and goals under supervision of a licensed therapist.
• Communicate findings to residents, families, and the interdisciplinary team.
• Schedule therapy sessions with team members.
• Document services and ensure CMS standards are met.
