Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and/or wound care, and develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• May involve standing, walking, lifting, transferring, and repositioning of patients and equipment as needed.
• Administer procedures as part of the rehabilitation plan (manual techniques, ambulation, therapeutic exercises, modalities, devices, wound care).
• Provide consultation and counseling to patients, families, caregivers and other providers regarding physical disorders.
• Generate comprehensive discharge summaries to ensure regulatory and clinical compliance.
