Job summary

Omega Senior Living in Seward, NE is seeking an on-site Health and Wellness Coordinator to support the Health and Wellness Director in delivering clinical services and maintaining resident care plans.

You will supervise personal service assistants, coordinate with the Executive Director, and ensure accurate documentation of medications and services while promoting resident dignity and safety across the residence.

Qualifications

• Must be a licensed LPN/LVN or RN per state rules.
• Prefer experience in assisted living or skilled nursing.
• Minimum 1 year of relevant nursing experience.

Responsibilities

• Support Health and Wellness Director in clinical services across the residence.
• Assist in service planning and maintenance of care plans.
• Supervise personal service assistants to meet residents' needs.
• Ensure accurate documentation of care and medications.
• Act as resident advocate in medical and social situations.
• Promote infection control procedures among staff.
• Identify resident concerns and follow up for resolution.
• Be available during night shifts as needed.

Health and Wellness Coordinator

Location: Seward, NE 68434

Our Assisted Living Health and Wellness Coordinator will support and take direction from our Health and Wellness Director in all health care services throughout the residence. Participate in service planning, as well as renewal and maintenance of these plans as needed. Cooperating with the Health and Wellness Director and the Executive Director in their duties to supervise, oversee, and coordinate the work of personal service assistants to meet Resident’s needs. Provide clinical services to Resident’s and assist in training personal service assistants.

Essential Duties:

• Adhere to and convey a philosophy of supporting dignity, privacy, independence, choice, and individuality for Residents.


• Assist current HWD in coordinating, in cooperation with the Executive Director, ancillary health care and health education services for Residents.


• Aid that all required documentation is completed accurately and thoroughly, to include service and medication delivery, and that all records required by regulatory agencies or company policy are maintained effectively and according to guidelines.


• Act as Resident advocate in medical/social situations involving Resident and/or designated agents, physician, or other agencies under the direction of the current HWD.


• Promote infection control procedures to be known and followed by all Associates. Track trends for Quality Management Review


• Assist in verifying physician orders, move-in/move-out information and initiate appropriate action and follow-up.


• Assist in identifying Resident problems, concerns, or issues and providing appropriate follow-up to ensure resolution.


• Nighttime HW Coordinator must be immediately available and awake.

Education/Experience:

• Must be a licensed LPN/LVN or RN (as required by state guidelines) and any necessary examination(s) in accordance with laws of this State. Prefer prior experience in AL or SNU. Prefer prior experience as a LPN/LVN or RN for at least 1 year.
