Job summary

Omega Senior Living in McCook, NE seeks a Health and Wellness Coordinator to support health care services under the Health and Wellness Director, coordinating care plans and supervising personal service assistants to meet residents' needs.

This position requires state licensure (LPN/LVN or RN) with preferred AL or SNF experience; you will ensure documentation accuracy, advocate for residents, and promote infection control while aligning with regulatory requirements.

Qualifications

• Licensed LPN/LVN or RN per state guidelines.
• Prefer AL or SNF experience.
• At least 1 year of LPN/LVN or RN experience.

Responsibilities

• Adhere to dignity, privacy, independence, choice, and individuality for Residents.
• Assist HWD and Executive Director in coordinating ancillary health care and education services.
• Ensure accurate documentation for service delivery, medication, and regulatory records.
• Advocate for Residents in medical/social situations under HWD guidance.
• Promote infection control practices and track quality trends.
• Verify physician orders and move-in/move-out information with appropriate follow-up.
• Identify Resident problems and provide timely follow-up.

Health and Wellness Coordinator

Our Assisted Living Health and Wellness Coordinator will support and take direction from our Health and Wellness Director in all health care services throughout the residence. Participate in service planning, as well as renewal and maintenance of these plans as needed. Cooperating with the Health and Wellness Director and the Executive Director in their duties to supervise, oversee, and coordinate the work of personal service assistants to meet Resident's needs. Provide clinical services to Resident's and assist in training personal service assistants.

Essential Duties:

• Adhere to and convey a philosophy of supporting dignity, privacy, independence, choice, and individuality for Residents.
• Assist current HWD in coordinating, in cooperation with the Executive Director, ancillary health care and health education services for Residents.
• Aid that all required documentation is completed accurately and thoroughly, to include service and medication delivery, and that all records required by regulatory agencies or company policy are maintained effectively and according to guidelines.
• Act as Resident advocate in medical/social situations involving Resident and/or designated agents, physician, or other agencies under the direction of the current HWD.
• Promote infection control procedures to be known and followed by all Associates. Track trends for Quality Management Review
• Assist in verifying physician orders, move-in/move-out information and initiate appropriate action and follow-up.
• Assist in identifying Resident problems, concerns, or issues and providing appropriate follow-up to ensure resolution.
• Nighttime HW Coordinator must be immediately available and awake.

Qualifications:

Education/Experience:

• Must be a licensed LPN/LVN or RN (as required by state guidelines) and any necessary examination(s) in accordance with laws of this State. Prefer prior experience in AL or SNU. Prefer prior experience as a LPN/LVN or RN for at least 1 year.
