Job summary

Gift of Hope Organ & Tissue Donor Network seeks an Outreach/Donor Coordinator to evaluate hospital referrals for donation potential and maintain referrals under follow-up. You will educate hospital staff, collaborate with medical teams, and support families through donation decisions.

Responsibilities include responding to dispatches, reviewing medical records, and conducting PCCs to align goals of care with family support. Strong communication and empathy are essential.

Qualifications

• Minimum of one year in Allied Health, graduate RN, or some previous OPO experience
• Excellent knowledge of or training in medical terminology required

Responsibilities

• Respond to dispatches, on-sites and phone follow-ups within established timeframes
• Adhere to dispatch instructions and hospital policy information
• Conduct evaluations of potential organ and tissue donors through review of hospital records
• Review hospital charts and current records of potential donors; document findings and follow-up plans
• Collaborate with hospital medical team to maximize the donation opportunity
• Coordinate and conduct Patient Care Conferences with medical team and service lines involved in care
• Provide real-time education on donation to hospital staff while on-site
• Develop solutions to issues that impede the donation process
