Job summary

Leisure Vale Assisted Living is seeking a Case Manager to support Veterans with case management and residential services. The role focuses on assisting with service plans, housing goals, and independent living, while connecting residents to VA and community resources.

The ideal candidate has knowledge of Veteran services, strong communication skills, and a compassionate, patient approach. First-Aid certification and a background check are required.

Qualifications

• High School Diploma or GED is required.
• Bachelor’s degree in Social Work or a related field is preferred.
• Two years of related experience is preferred.
• Military or Veteran experience is strongly preferred.
• Knowledge of Veteran services and community resources is a plus.
• Valid First-Aid certification required.
• Criminal background check required.
• Must be at least 18 years old.

Responsibilities

• Provide case management and residential support to Veterans.
• Assist residents with service plans, housing goals, and independent living skills.
• Connect Veterans with VA, medical, social, educational, and community resources.
• Assist with housing searches, appointments, outreach, and discharge planning.
• Facilitate structured groups and living skills activities.
• Monitor residents’ progress and well-being.
• Maintain accurate documentation and progress notes.
• Collaborate with VA personnel, families, providers, and the care team.
• Assist residents with daily living activities when needed.
