Job summary

Kaiser Permanente in Washington, DC seeks an experienced Inpatient Social Work Case Manager. This role coordinates patient care with the health care team, conducts comprehensive psychosocial assessments, and guides access to community resources and government programs.

Requires an MSW, DC MD VA licenses or eligibility, and at least 1 year of clinical social work in health care. A $7,500 sign-on bonus is offered; some responsibilities may require weekend coverage.

Qualifications

• MSW required with current licensure in DC/MD/VA considerations.
• Experience coordinating care, discharge planning, and community referrals.
• Knowledge of regulatory issues for the Mid-Atlantic area preferred.
• Experience with Windows-based software preferred.

Responsibilities

• Collaborates with health care team to coordinate cost-effective services.
• Completes psychosocial assessments and plans of care.
• Facilitates access to local/state/federal programs and resources.
• Maintains documentation per organizational standards.
• Supports families and patients, including end-of-life discussions.
• Performs other related duties and may float to cover staffing.
