Qualifications

• Minimum one (1) years of clinical social work experience in a health care setting required.
• Masters degree in social work (MSW) required.
• Licensed Independent Social Worker (District of Columbia) within 6 months of hire OR Licensed Graduate Social Worker (District of Columbia) within 6 months of hire
• Licensed Master Social Worker (Maryland) within 6 months of hire
• Licensed Master's Social Worker (Virginia) within 6 months of hire
• This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire.

Responsibilities

• Collaborate with health care team to coordinate care and access resources.
• Complete psychosocial assessments to evaluate goals, support systems, and resources.
• Develop and monitor a plan of care with the nurse case manager.
• Identify and advocate for community resources to meet care plans.
• Document case management activities per standards.
• Analyze outcomes to improve program effectiveness and reduce costs.
• Facilitate applications for local/state/federal programs (Medicaid, Medicare).
• Provide supportive counseling and education to members, families, caregivers, and staff.
• Schedule and coordinate family meetings; assist with guardianship paperwork when needed.
• Inpatient: complete UAIS form for Virginia Medicaid when required.
• Perform other related duties as assigned.
