Job summary

Trinity Health PACE in West Springfield, MA is seeking an experienced Registered Nurse supervisor to coordinate and supervise nursing services for our participants, with care delivered in the day center or at home.

The role requires leadership, participation in IDT care planning, QAPI, and ensuring regulatory compliance. Expect weekend rotation and on-call duties, with a competitive hourly pay and a comprehensive benefits package.

Qualifications

• Graduate of an accredited nursing degree program with active MA RN license.
• BSN preferred.
• Active CPR certification required.
• Minimum three years healthcare experience with geriatrics emphasis.
• Previous supervisory experience required.
• Valid driver's license, insurance, and access to own vehicle for home care assignments.
• Superior written and verbal communication skills.
• Proficiency with MS Office.
• Must be flexible, able to prioritize workload and exercise independent judgment.
• Local travel required within the general area.

Responsibilities

• Coordinate and supervise nursing services for PACE participants.
• Participate in interdisciplinary team (IDT) care planning.
• Participate in QAPI, budget planning and survey preparedness.
• Supervise center and/or in-home staff including orientation and performance appraisal.
• Facilitate integration of new participants into the health care delivery system.
• Ensure adherence to all federal and state regulations.
• Review participant medical records to ensure timely staff documentation.
• Counsel and guide participants and families regarding service needs; make referrals.
• Participate in the administrative on-call rotation.
• Communicate with weekend and afterhours on-call staff.
• Serve on committees supporting daily operations.
• Participate in education program preparation and presentations.
