Job summary

Trinity Health PACE in West Springfield, MA is hiring a Clinic Manager responsible for oversight and delivery of nursing services to PACE participants in community, day center, or primary care clinic settings. You will lead clinical operations, coordinate with the primary care team, and ensure high-quality, coordinated care.

The role emphasizes strategic leadership, budget management, and staff development within a mission-driven environment.

Qualifications

• Current, active RN license in the state.
• BSN degree is required.
• Active CPR certification.
• Minimum of 3 years of healthcare experience, with emphasis in geriatrics.
• At least 1 year of experience working with frail or elderly populations (or training on hire).
• Strong leadership, conflict management, and decision-making skills.
• Excellent interpersonal and communication skills, both written and verbal.
• Proficiency in Microsoft Office Suite and familiarity with electronic mail and calendaring systems.
• High level of personal accountability and independent decision-making ability.
• Strong organizational and problem-solving skills.
• Ability to prioritize tasks and adapt to changing work priorities.
• Critical thinking and analytical skills for effective decision-making.

• Ensure compliance with laws, regulations, and policies.
• Participate in disaster recovery and business continuity planning.
• Attend required training and sessions.
• Oversee clinic operations as the central PACE hub for participant care.
• Participate in interdisciplinary team meetings and care planning.
