Job summary
Trinity Health PACE in Massachusetts seeks an experienced RN nursing leader to coordinate and supervise nursing services for participants in the PACE program. The role involves center and home-care assignments, and participation in interdisciplinary planning and quality initiatives.
The position requires supervisory experience, active RN licensure in the state, CPR certification, and a focus on patient-centered care with strong communication skills. Local travel and on-call duties are included.
Qualifications
- Graduate of accredited nursing program with active RN license in state.
- BSN preferred.
- Active CPR certification required.
- Minimum three years healthcare experience with geriatrics.
- Previous supervisory experience required.
Responsibilities
- Oversee center operation as the focal point of the PACE model.
- Participate in IDT and care planning.
- Participate in QAPI, budgeting, and survey readiness.
- Supervise center and home care staff, including training and evaluation.
- Ensure adherence to federal and state regulations.
- Review participant records for timely staff documentation.
- Communicate with participants and families regarding needs and referrals.
- Participate in on-call rotation and weekend coverage.
