St. Luke’s in Idaho is seeking an RN Care Coordinator to join our Care Management team. You will build relationships with patients and families, assess needs, educate, support self-management, and coordinate resources across the health system and community.

Embedded in Eagle Internal Medicine, Caldwell, Greenhurst, and Caldwell clinics, you will collaborate with primary care providers and the care team to deliver coordinated, patient-centered care in a fast-paced environment.

Qualificações

• Bachelor's of Nursing (BSN)
• 2-3 years of clinical nursing experience; ambulatory care, care management, triage or population health preferred
• Strong clinical assessment, critical thinking and communication skills
• Ability to collaborate with multidisciplinary teams and manage competing priorities in a fast-paced environment
• Active RN license
• BLS certification
