Job summary

Premise Health in Foster City, CA is hiring a Full-Time Physical Therapist to provide care to employees and their dependents at our Health Center. Hours are Monday–Friday, 7:30am–4:30pm.

You will work with a multidisciplinary team and practice to modern standards, promoting direct access to physical therapy and delivering patient-centered, high-quality care. You will document and report to onsite and community referral sources.

Qualifications

• Licensed to practice physical therapy in the relevant state.
• Degree from an accredited Physical/Occupational Therapy program.
• CPR (BLS) certification with hands-on evaluation; meets AHA standards.
• Current board certification (if applicable).
• Minimum three years of outpatient PT experience.

Responsibilities

• Provide care to employees and dependents at the Foster City Health Center.
• Collaborate with doctors, NPs, PAs, nurses and wellness staff.
• Promote direct access and deliver care at the highest skill level.
• Document care and report to referral sources; participate in meetings.
• Engage in injury prevention and health promotion programs.
