Job summary

Premise Health Inc. seeks a Part Time Physical Therapist to provide care to client employees in our Health Center located in Oklahoma City, OK.

The role is scheduled for Friday 7:30 AM - 4:30 PM and emphasizes high-quality, patient-centered care in an onsite clinic setting. Qualified candidates will have state licenses, CPR certification, and at least three years of outpatient PT experience, with a degree from an accredited PT program.

Qualifications

• Active state licenses/certifications to practice.
• Current board certificate (if applicable).
• Degree from accredited Physical/Occupational Therapy School.
• CPR (BLS for the Healthcare Provider) certification with hands-on skills evaluation.
• At least three years of outpatient PT experience.

Responsibilities

• Provide PT care to onsite employees and dependents.
• Collaborate with Health Center staff to promote health and wellness.
• Follow APTA standards and evidence-based treatment planning.
• Maintain patient-centered documentation and reporting to referral sources.
• Engage in continuing competence activities and quality improvement.
