Job summary

Premise Health in Burbank, CA is seeking a Full Time Physical Therapist to provide care to client employees and dependents at our Health Center. Start date targeted for end of March 2027.

You will collaborate with physicians, nurses and wellness staff to promote health and deliver evidence-based treatment plans. Requirements include state licensure, a degree in Physical Therapy, CPR certification, and at least three years of outpatient PT experience.

Qualifications

• All state licenses/certifications to practice.
• Current board certificate (if applicable).
• Degree from accredited Physical/Occupational Therapy School.
• CPR (BLS for Healthcare Provider) with hands-on evaluation.
• At least three years’ outpatient PT experience.

Responsibilities

• Collaborate with doctors, NPs, PAs and wellness staff to promote health at onsite Health Center.
• Provide PT services per APTA standards with patient-centered treatment planning.
• Promote direct access to PT services and practice at high skill level.
• Document and report to onsite and community referral sources.
• Engage in continuing competence activities including peer review.
• Work with stakeholders to promote employee safety in the workplace.
• Develop injury prevention and health promotion programs for onsite employees.
• Teach proper body mechanics at workstations.
• Assess and correct ergonomics.
• Develop programs like Weekend Warrior, Health Back Programs, Stretch and Flex.
• May require other duties as assigned.
