Job summary

Minimum High School Diploma with 2+ years in a high-volume healthcare setting; Bachelor’s degree preferred.

Qualifications

• High School Diploma is required.
• Minimum of two years’ experience in a high-volume complex healthcare environment.
• Excellent telephone and personal communication skills required; proficient with computer systems.
• Ability to multi-task and prioritise is crucial.
• Compassionate listening and a strong interest in patient interaction.
• Experience with and commitment to customer service and patient confidentiality.
• Bachelor's degree preferred but not required for all candidates.

Responsibilities

• Schedule appointments and coordinate referrals for testing, imaging, procedures, surgeries, social work, therapy services, labs, and other care.
• Obtain authorizations, medical records, slides, specimens, and other items for care coordination.
• Follow up on results with appropriate services and share with patients and community physicians as needed.
• Provide education to patients and caregivers about the care plan and follow up to ensure timely arrival for appointments.
• Handle patient and caregiver inquiries regarding appointments and coordination of care.
• Review clinic schedules and templates to optimize daily scheduling and rescheduling as needed.
• Provide navigation and care coordination across UCM Network Sites.
