Job summary

City of Hope is seeking an experienced Registered Nurse, Oncology Nurse Navigator to join our on-site team in Illinois. The role focuses on disease management, symptom monitoring, patient education, and coordination of care with a multidisciplinary team.

You will triage patient issues, document in the EMR, and collaborate with physicians, families, and external agencies to optimize treatment plans and patient satisfaction. Requires IL RN license and BSN or higher; BLS is required.

Qualifications

• 3-5 year clinical nursing experience.
• Current IL RN license.
• BSN or higher preferred.
• BLS Certification through American Heart Association.
• OCN or Bio/Chemo certification preferred.

Responsibilities

• Provide disease management with oncology focus; monitor symptoms and educate patients and families.
• Document assessments and interventions in the EMR in a timely manner.
• Collaborate with the provider team to manage patient care and coordinate services.
• Triages patient issues in person or by phone; use orders and team consultation as needed.
• Educate patients using appropriate resources and document education in the EMR.
• Support care coordination and multidisciplinary plans of care; set patient-centered goals.
