Job summary

UChicago Medicine is seeking an Oncology Nurse Navigator for our Tinley Park clinic to coordinate care across the cancer treatment pathway. This 100% onsite role requires collaboration with the Oncology team and base in the greater Chicagoland area.

You will assess patient needs, facilitate education, and support discharge planning while ensuring communication with families and multidisciplinary teams. Experience in hematology/oncology nursing is essential.

Qualifications

• Registered Nurse with an Illinois license and BSN.
• Minimum two years hematology/oncology clinical nursing experience.
• Excellent interpersonal skills and ability to adapt to changing work situations.
• General computer skills and familiarity with electronic medical records.

Responsibilities

• Act as liaison for patients, families and the multidisciplinary team.
• Assess clinical, emotional, psychosocial, and financial patient needs.
• Educate patients/families on disease, treatments, side effects, and warning signs.
• Coordinate discharge planning with families and home care teams.
• Develop and coordinate patient education materials and care pathways.
• Participate in tumor conferences and document recommendations.
• Support development of feasible treatment plans and act as escalation point for intake coordinators.
