Job summary

Lutheran Social Service of Minnesota seeks a Designated Coordinator for on‑site leadership in residential programs in Grand Rapids, MN. You will coordinate service delivery, ensure program quality, and oversee teams to meet the needs of 4+ individuals under Minnesota 245D compliance.

The role requires a human services degree pathway with relevant direct care experience and supervisory training. The position offers a $1,000 hiring bonus and a full‑time schedule.

Qualifications

• Bachelor’s degree in a human services field plus 1 year of full‑time direct care with disabilities or older adults.
• Associate degree plus 2 years of direct care experience.
• Diploma in a human services field plus 3 years of direct care experience.
• 50 hours of education/training in human services and disabilities plus 4 years of direct care experience.

Responsibilities

• Program Oversight: direct responsibility for homes serving 4+ individuals with meals, supplies, and finances.
• Service Delivery: coordinate person‑centered outcomes and safeguard health and safety.
• Team Leadership: supervise and coach direct support professionals.
• Compliance & Quality Assurance: monitor 245D compliance and policies.
• Collaboration: work with analysts, therapists, clinicians, and stakeholders.
• Direct Care: provide hands‑on support for 16+ hours weekly.
• Administrative Duties: manage budgets, records, and regulatory communications.

Education

Bachelor’s degree in a human services field

1 year of full‑time direct care experience with individuals with disabilities or adults age 65+

Associate degree in a human services field

Diploma in a human services field from an accredited postsecondary institution

50 hours of education/training in human services and disabilities + 4 years of direct care experience
