Job summary

Lutheran Social Service of Minnesota is seeking a Designated Coordinator to oversee service delivery in residential settings for 4+ individuals. You will ensure compliance with state standards and lead a direct support team to promote independence and safety.

The role requires meeting Minnesota Statute 245D qualifications and may involve after‑hours readiness for emergencies. Expect onsite supervision, budget oversight, and collaboration with health professionals to support meaningful outcomes.

Qualifications

• Bachelor’s degree in a human services field + 1 year of full‑time direct care experience with individuals with disabilities or adults age 65+.
• Associate degree in a human services field + 2 years of full‑time direct care experience.
• Diploma in a human services field from an accredited postsecondary institution + 3 years of full‑time direct care experience.
• Minimum of 50 hours of education/training in human services and disabilities + 4 years of full‑time direct care experience under qualified supervision.

Responsibilities

• Program Oversight: direct responsibility for one or more residential homes serving 4+ individuals; ensure healthy meals, supplies, accounts payable, etc.
• Service Delivery: coordinate person-centered outcomes while safeguarding health and safety.
• Team Leadership: supervise direct support professionals, including training and performance management.
• Compliance & Quality Assurance: monitor adherence to 245D requirements and policies.
• Collaboration: work with clinicians and professionals to align support.
• Direct Care: provide minimum 16 hours of hands‑on support, with extra hours as needed.
• Staffing & Shift Coverage: manage team schedules and coverage.
• Administrative Duties: manage budgets, documentation, and communications with regulators.
