Job summary

Daymark Health Inc. is seeking a Health Partner to support integrated, whole-person care. This onsite role in Long Beach, CA, involves coordinating social and logistical aspects of care, contacting patients by phone, video, and sometimes in-home visits.

Qualifications

At least 3 years of experience in healthcare navigation or patient support.
Proficiency with Slack, Zoom, Google Suite, and EHR systems.
Ability to prioritize tasks and act quickly.
Comfort with change and ambiguity while delivering quality work.
Strong relational and communication skills with diverse populations.

Responsibilities

Conduct social needs assessments and connect patients with community resources.
Support development of patient care plans and participate in case conferences.
Serve as the primary contact for virtual visit readiness and basic tech support.
Coordinate and schedule care, follow-up visits, and referrals.
Prepare structured charts to ensure visit readiness with accurate data.
Document social interventions and care coordination in the EHR promptly.
