Job summary

The Case Manager at The Village plays a critical role in supporting individuals experiencing homelessness by providing comprehensive, client-centered case management services. This role assesses client needs, develops individualized service plans, links clients to housing, health, and supportive services, and promotes stability and well-being within a trauma-informed framework.

Working 100% onsite at The Village, you will collaborate with a multidisciplinary team, maintain HMIS documentation,

Qualifications

• Minimum of one year of experience in case management.
• Knowledge of issues faced by homeless population; trauma-informed approach.
• Ability to drive to multiple sites with a valid CA license.

Responsibilities

• Manage a caseload of clients with active case management.
• Lead housing-related case management with active housing plans.
• Coordinate with housing, health, and social services resources.
• Maintain client confidentiality and HMIS documentation.
