Qualifications

• Licensed or certified clinical staff with US work authorization.
• Experience coordinating patient care in a clinical setting.
• Strong bedside manner and patient-centered approach.
• Proficient use of EMR systems and Microsoft 365 tools.

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans.
• Coordinate care using EMR for continuity and timely follow-up.
• Schedule provider visits and manage clinic workflow.
• Collaborate with clinical staff to ensure integrated care.
• Communicate patient needs and changes promptly to leaders.
