Ennoble Care in Pensacola, FL is hiring a full-time Care Coordinator, Embedded at Partnered-Dialysis Clinics. This on-site role acts as a key liaison within a dialysis facility, coordinating care plans, scheduling provider visits, and ensuring timely follow-up with patients, families, and the clinical team.

The ideal candidate is compassionate, highly organized, and capable of maintaining accurate documentation while communicating clearly across teams.

Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted clinical certification or licensure such as RN/LPN/CNA/MA/CMA/PCT.
• Dialysis-specific certification is highly preferred (CDT/CHT/CCHT).
• Experience managing provider schedules and coordinating patient appointments in a clinical setting preferred.
• Minimum 1 year dialysis/nephrology patient care experience preferred.

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans and transitions.
• Coordinate care using the EMR to ensure continuity and timely follow-up.
• Schedule patient visits for providers and support daily clinic workflow.
• Collaborate with clinicians, dialysis staff, and care team for integrated patient care.
• Serve as liaison between providers, patients, families, and facility staff with proactive communication.
• Document accurately meeting healthcare regulations and organizational standards.
• Demonstrate professionalism and sound judgment amid multiple priorities in a clinical setting.
• Utilize Microsoft Teams, email, and in-person communication effectively.
