Job summary

Ennoble Care is seeking a full-time Care Coordinator, Embedded at Partnered-Dialysis Clinics in Lehigh, PA. This on-site role engages patients, coordinates care plans with Ennoble providers and facility staff, and ensures timely follow-up.

You will maintain accurate documentation, support clinic workflow, and communicate clearly with patients and families. Qualified candidates will have active clinical licensure or certification and experience in dialysis settings.

Qualifications

• Legally authorized to work in the United States.
• Active clinical licensure or certification in a relevant field.
• Dialysis experience preferred; care coordination within a clinical setting valued.
• Experience coordinating patient appointments and provider schedules.
• Strong bedside manner with patient-centered approach.

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans and follow-ups.
• Coordinate care using EMR to ensure continuity and timely follow-up.
• Schedule patient visits and support daily clinic workflow.
• Collaborate with providers, dialysis staff, and multidisciplinary team.
• Communicate patient needs and changes to the clinical leader promptly.
• Maintain accurate documentation per standards and regulations.
• Demonstrate professionalism and manage multiple priorities in a fast-paced environment.
