Qualifications

Responsibilities

• Perform intake assessments for new residents.
• Develop individualized Service Plans for residents.
• Conduct Wellbeing Checks and Emergency Safety Checks.
• Facilitate therapeutic groups and community activities.
• Ensure compliance with policies and procedures.

Education

High School Diploma + 4 years’ experience

AA degree + 3 years’ experience

CHW Certificate/CADC + 2-4 years’ experience

MA/MS degree + 1 year experience
