Job summary

Northwestern Medicine Catherine Gratz Griffin Lake Forest Hospital is seeking a Triage Nurse to join our team. The RN will provide clinically expert triage within a physician’s practice, assess patient acuity, educate patients and staff, and act as a patient advocate while coordinating care across providers.

Required is an accredited RN program grad, 3+ years of hospital or outpatient experience, an Illinois RN license, and CPR certification.

Qualifications

• Graduate of an accredited RN program.
• Minimum 3 years’ hospital or outpatient experience.
• Illinois RN license current.
• CPR certification.
• BSN preferred.
• Specialty Certified.
• At least 5 years’ experience in a hospital or outpatient setting.
• Spanish speaking.

Responsibilities

• Establish positive first impressions with patients and customers, acting as patient advocate.
• Triage all incoming calls or messages, prioritizing and resolving, involving the provider as appropriate.
• Document all communications in the electronic medical record.
• Schedule patient visits when telephone assessment indicates this.
• Assist in coordinating appointments with NM specialists.
• Coordinate emergency and hospital transfers, including paperwork and calls.
• Administer medications and treatments per nursing standards as ordered by the provider.
• Provide education to patients and staff concerning clinical issues.
• Assist in coordinating clinical staff coverage and OSHA/CDC guideline adherence.
• Assist in orientation of new staff members.
• Perform other duties as assigned; may include process improvement projects.
• Float to other ICCs/departments as needed.
