Job summary

University Hospitals is seeking a Social Worker 2 for the Seidman Cancer Center, to join an outpatient cancer care team in Mentor/Concord. The role supports oncology patients, coordinating psychosocial care and discharge planning within a collaborative environment.

Responsibilities include psychosocial assessment, age-appropriate care, and development of comprehensive plans, with liaison to community resources and UH services. Opportunities for growth and education are highlighted.

Qualifications

• Experience performing psychosocial assessments and interventions.
• Ability to develop patient- and family-centered discharge plans.
• Strong collaboration with a multidisciplinary care team.

Responsibilities

• Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
• Provides age specific care to patients/families in all age groups.
• Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
• Develops plans with attention to individual patient and family goals.
• Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
• Communicates/collaborates with the work team using a multidisciplinary approach.
• Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
• Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.
