University Hospitals in Mentor, OH is seeking a Social Worker 3 LISW for outpatient oncology. The role collaborates with the inter-disciplinary care team to assess psychosocial needs and support patients and families with discharge planning.

Requires a Master's in Social Work and LISW licensure; oncology or chronic disease experience preferred. This 32-hour-per-week position relocates to Concord in November, with Monday through Friday daytime shifts and strong benefits.

Qualifications

We currently offer an opportunity for an outpatient, Part Time, days, Social Worker to support oncology patients in ouroutpatient cancer centerin Mentor.

A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.

What You Will Do
  • Provides psychosocial assessment / intervention to assist patients/families (including clinically complex). Provides age specific care to patients/families in all age groups.
  • Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs. Develops plans with attention to individual patient and family goals. Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
  • Communicates/collaborates with the work team using a multidisciplinary approach.
  • Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
  • Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.

  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Education
  • Master's Degree from an accredited school of Social Work (Required)
Work Experience
  • Hospital/Health Agency experience. (Preferred)
Knowledge, Skills, & Abilities
  • Theoretical and clinical knowledge regarding health care delivery (Required proficiency)
  • Multi-tasks and prioritizes work (Required proficiency)

Licenses and Certifications
  • Licensed Independent Social Worker (LISW) (Required)

Preferred Qualifications/Position Details

Must have previous experience, preferably in oncology or chronic diseases. This Social Worker will support all patients in a large, outpatient cancer center. Responsibilities will include performing psychosocial assessments and distress screens, supporting patients with emotional and mental health issues, providing various social service resources, etc. This position is in Mentor but will be relocating to Concord in November. Position is 32 hours per week, Monday-Friday.
