Job summary

University Hospitals Pain Management in Mentor, OH, is seeking a part-time Social Worker to support oncology outpatients and their families. The role involves psychosocial assessment, discharge planning, and coordination with the UH network to ensure continuous, patient-centered care.

You will collaborate with a multidisciplinary team, provide linkage to community resources, and mentor junior staff while maintaining PHI compliance and pursuing ongoing professional development.

Qualifications

• Collaborates with health care team to coordinate timely psychosocial assessments and discharge planning.
• Performs psychosocial assessment/intervention for patients and families, including clinically complex cases.
• Develops and documents comprehensive discharge plans with attention to patient and family goals.
• Links patients to community resources and UH network services to facilitate care.
• Maintains patient confidentiality and adheres to PHI policies.

Responsibilities

• Provides psychosocial assessment/intervention to assist patients and families across age groups.
• Develops, documents, and implements comprehensive psychosocial discharge plans.
• Coordinates linkage to community resources and UH services; acts as liaison with community agencies.
• Collaborates with the multidisciplinary team to support coordinated patient care.
• Supports recruitment, orientation, mentoring, and education of new staff or social work interns.
• Performs other duties as assigned.
