University Hospitals in Mentor/Concord area seeks a Social Worker for their Seidman Cancer Center. This outpatient, full-time dayshift role supports oncology patients and families, coordinating psychosocial assessments, discharge planning, and community resource linkage with a multidisciplinary team.

Requires a Master’s Degree in Social Work and Licensed Social Worker (LSW). LISW is preferred within one year.

A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.

What You Will Do

• Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
• Provides age specific care to patients/families in all age groups.
• Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
• Develops plans with attention to individual patient and family goals
• Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
• Communicates/collaborates with the work team using a multidisciplinary approach.
• Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
• Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.

Position Details/Preferred Qualifications:

Post-Master’s experience is strongly preferred. Previous work in oncology or chronic diseases is also preferred. This Social Worker will support all oncology patients in a large, outpatient cancer center. This position supports the significant psychosocial and social service needs (including transportation, mental health, grief, housing, utility, grant applications, referral to community resources, etc.) for an increasing patient population. This will also help ensure treatment compliance, fewer missed appointments, and reduction in hospital re-admissions. This position is in Mentor but will be relocating to Concord in November. The schedule is Monday-Friday, weekdays.
