Job summary

University Hospitals in Mentor, OH is seeking a Social Worker 3 LISW for outpatient oncology. The role collaborates with the inter-disciplinary care team to assess psychosocial needs and support patients and families with discharge planning.

Requires a Master's in Social Work and LISW licensure; oncology or chronic disease experience preferred. This 32-hour-per-week position relocates to Concord in November, with Monday through Friday daytime shifts and strong benefits.

Qualifications

• Master's Degree from an accredited school of Social Work.
• Licensed Independent Social Worker (LISW) required; LISW-S preferred.
• Oncology or chronic disease experience preferred.
• Excellent communication and ability to work autonomously.

Responsibilities

• Performs psychosocial assessment/intervention for patients and families.
• Develops, documents and implements comprehensive discharge plans.
• Coordinates with care teams and links to community resources.
• Provides guidance and mentoring to staff and social work interns.
• Ensures adherence to PHI/privacy and UH policies.
