University Hospitals in Cleveland is seeking a Social Worker 2 to support the Sickle Cell & Benign Hematology Diseases outpatient program. You will collaborate with oncology teams to assess psychosocial needs, plan discharges, and connect patients to community resources, housing, transportation, and insurance assistance.

The role emphasizes interdisciplinary teamwork, patient-centered care, and ongoing education within the Seidman Cancer Center, with a focus on sickle cell and hematology

Formación

• Master's Degree in Social Work required.
• Licensed Social Worker (LSW) required; LISW preferred within one year.
• Hospital/Health Agency experience preferred.

Responsabilidades

• Performs psychosocial assessment and intervention for patients and families.
• Develops and documents comprehensive discharge plans.
• Coordinates with the health care team and community resources.
• Mentors staff and social work interns; supports education and development.
• Performs other duties as assigned and ensures PHI compliance.
